• Who qualifies for cardiac rehab. Learn which heart conditions and procedures typically qualify, so you can talk with your provider about whether cardiac rehab is right for you.

• Never heard of cardiac rehab? You’re not alone. Many eligible patients are never told about it. Learn how cardiac rehab uses exercise, education, and lifestyle support to help improve heart health.

• Getting a referral. Cardiac rehabilitation requires a referral from a healthcare provider. You can request a referral from your own provider, or schedule a visit with a primary care physician through our primary care program, who can evaluate whether cardiac rehab is right for you and write the referral when appropriate.

• Getting connected. Once you have a referral, we’ll share the resources you need to connect with a cardiac rehab program near you. Enrollment depends on a provider’s referral and insurance approval.
