Topics: Primary & Secondary Cardiovascular Prevention  

This course will focus on primary and secondary cardiovascular disease prevention. You will learn how to MAKE decisions to improve heart function and achieve and maintain heart health post -MI.
Education

  • A proactive education plan that puts you in the driver's seat. We educate you and teach you how to take control of your health!

  • Learn about Cardiac Rehabilitation

  • Presentation of evidence-based research supporting the health advantages of enrolling in Cardiac Rehabilitation

  • We will educate you about your candidacy for cardiac rehab

  • We will educate you about the diagnoses accepted 

  • My doctor never mentioned cardiac rehab why should I consider enrolling? We will educate you about cardiac rehab and the application of non-pharmacological interventions to improve your heart health

  • Cardiac Rehabilitation requires a referral from your provider. We will provide you with the resources required to get you connected with the right rehab team (enrollment is contingent upon insurance approval)

 

Note: This is an EDUCATION service ONLY. We provide this service because although cardiac rehabilitation has demonstrated benefits, fewer than 20% of eligible patients participate. We provide the facts about rehab to help you decide if it is a viable option for you. You need support to rehabilitate, recover and realize a new life after a cardiac event or diagnosis.- American Heart Association

    Is This Service Right For Me? 
 
This plan is best suited for the client with a current diagnosis: I have been diagnosed by my physician/nurse practitioner or physician assistant with one or more of the following: stable angina, heart attack, heart failure, coronary artery bypass grafting, heart valve repair or replacement, heart or lung transplant, angioplasty with or without stent, peripheral artery disease, pulmonary hypertension and I would like to learn more about how cardiac rehabilitation can improve my heart health. Am I eligible?
