Phase 2: Self-Management

• Your health specialist will help you foster self-management skills helping you select the appropriate lab tests to address your health concerns.

• Your health specialist will help you foster self-management skills by thoroughly educating about scheduling the appropriate lab tests to track your progress

• Your health specialist will help you foster self-management skills by providing web scheduling assistance. Client will use tablet to schedule lab appointment

• Your health specialist will help you foster self-management skills by thoroughly educating you about the target range (clinical values) LDL, HDL, A1C, C-reactive protein required to achieve and maintain heart health

• After client submission, all orders are reviewed and approved by a licensed physician

• A cholesterol (blood test) will be scheduled 

• A diabetes (blood test) will be scheduled 

• A C-reactive protein (blood test) will be scheduled 

• Tests results will be reviewed by preventive health specialist 

• The POWER of education, client will learn if their numbers improved and appropriate educational courses will be recommended
